How to seek a referral to children's hearing services

Children can be referred to the audiology service if they are under a GP surgery in Southampton City or Southwest Hampshire. A full list of GP’s covered by this service can be found here, please check before sending a referral.

For patients

If you are concerned about your child’s hearing, you can ask for a hearing test with our service using the self-referral form below. 

You can also speak to your child’s GP, health visitor or school nurse for further advice and referral to audiology.

For health professionals

Health professionals referring children to our service can do so using the form below.

Please be aware that we provide both community-based clinics (Tier 2 – secondary care) and hospital-based clinics (Tier 3 – tertiary care).
