External referrals

Health professionals can refer a child to UHS for assessment for insertion of a gastrostomy by completing a referral form. Please complete the form electronically and follow the referral instructions on the form. If you wish to give further information please include a referral letter.

The quickest and most efficient way to send the referral is by email to the address on the form.
