Referring patients to the ILD service at Southampton

If you are a clinician seeking to refer a patient to be seen under the ILD service here in Southampton we ask that you send a completed version of the form below to ILDMDT@uhs.nhs.uk, along with the most recent clinical letter, the patient’s last three PFT results and any other information you feel relevant for review.
