Pathology contingency request forms

These forms should be used in conjunction with the stock item
specimen bags KFD009 (box of 1000),

• Download the PDF document from this page, then print, fold or cut as necessary prior to use
• Alternatively photocopy existing blank request form and use until new stock is ordered
• NB For microbiology a duplicate form is requested by the laboratory, therefore, if using microbiology forms try to implement an A4 form with two 'duplicate' microbiology request forms on one A4 sheet.
