Change or update your address
It is important that we have your latest contact details so we can inform you of any changes to your appointment(s) and/or conduct your appointment(s) over the telephone.
We often send text message and voicemail reminders of appointments. If you have received a letter from us at your current address, or have received text messages or voicemails from us about your appointment to your current numbers, you can be assured that we have your correct details. If any of these details have changed since your last contact with us they may need changing on our systems.
Changing your address
To change your address, you must be a UHS registered patient.
If you are a UHS patient, to request to change your address please email OutpatientAdminEnquiries@uhs.nhs.uk
Please use the following headings in the email:
- Email subject/heading: Patient request to change address
- Email body: Please provide:
- NHS/Hospital number (required):
- Full name (required):
- Date of birth (required):
- Current address (required):
- New address (required): - Attachments - please attach photo ID and a document with your new address on. If you are changing the address of a child, please also the child's birth certificate or passport and inform us who are you, and what your relationship to the child is.
Once your address has been changed, we will confirm this with you by email.
